Waiver/Consent

IJA Festival
July 16-22, 2012

All IJA festival registrants (or a parent/guardian of IJA festival registrants under 18) must sign the WAIVER/CONSENT to help the IJA continue to produce its annual festival and related videos and photographs.

No one will be admitted to the IJA festival without a signed Waiver/Consent form.

Festival registrants 17 or younger

If you are 17 years old or younger, print out the Parent/Guardian Waiver/Consent form, have your parent or guardian fill it out and sign it, and present it at the IJA table at the festival when you check in.

Anyone 17 or younger attending the festival without a parent/guardian must print out the Parent/Guardian Waiver/Consent form in advance, have it signed by a parent or guardian, and bring the signed form to the festival in order to be admitted.

Festival registrants 18 or older

If you are 18 years old or older, print out the Waiver/Consent form for adults, fill it out, sign it and present it at the IJA table at the festival when you check in.

Anyone 18 or older, or with a parent/guardian attending the festival, can fill out and sign the Waiver/Consent form at the festival. If you can print it in advance, though, please do and bring the filled-out form to the festival with you to save time at the registration table.
